Request Forms (under maintenance)

If you are looking for special assistance with your Canvas course, specific training regarding DE, or even wanting our department to research a nifty DE tool, this is the perfect location. Submit a form that pertains to your need and we will follow up with you as quick as possible.

Each form is created with Microsoft Forms; as such, you are required to login with your SBCCD credentials to access them. You must use your email that ends with @sbccd.cc.ca.us, but your password remains.

Course Copy/Export

Would you like your course copied or exported? Are you wanting to share your course with another instructor, but unsure how to do so? This is the form for you. Please note that the instructor who is sharing their must submit this request; it cannot be submitted by the instructor wanting to use the material.

Development/Program Shell

Interested in having a shell to work on your course material without affecting your actual Canvas course? Are you looking to create content for an entire department to access and work on? A development shell is the request for you. Please note that development and program shells are limited based on user and purpose.

Add DesignPLUS

Per the DE department, you must attend the training for DesignPLUS before it is added to your course. This is to ensure that you have a general idea of how DesignPLUS works and allows us the opportunity to see who is utilizing this amazing tool. If you have completed the DesignPLUS training at a different campus or in our district, please submit this quick request form. Add an Evaluator(s)

Are you being evaluated this semester? Make sure to submit this form so that way we can add your evaluator(s) to any course you are teaching this semester. You will be required to type the name and email of your evaluator as well as all classes that this evaluator needs to be added.

Add an SI/Tutor

Do you have an SI/Tutor for one or all of your classes this semester? This form will notify us of which courses we need to add the SI/Tutor(s) to so you can start the semester off with them present.

Specialized Training

Looking for a specific type of training? Not seeing something offered on the workshop list or would like to see again? Take the time to let us know what you would like to see in the future or what you would like us to train you on, specifically.

DE Tool/LTI

Is there a nifty tool that you found that fits perfectly with your class or that you have at another campus, but do not have here? Let us know what the tool is and we can research it for you! If the tool is approved and we have the budget for it, we can look into purchasing it for the district. Please note that this submission does not mean that the DE tool or LTI will be approved and paid for by the district.
