Faculty Evaluation, Request Forms (see Datatel documentation "Requesting Faculty Evaluation Forms" in the Faculty Information folder in Document Library and Process No. 66/SBVC and 221/CHC in Computing Services processes)

Administering the evaluation to the students, by the Division staff.
Source documents or communication with information needed for this process:
Receive e-mail from the Division secretaries or CHC`s Admin. Assistant confirming the sections Faculty Evaluation forms are to be generated for.
What is the end-result of this process, or the hand-off
Printed evaluation forms, boxed with report showing number of forms printed for each section.
What steps must be taken independent of the computer system (Manual Process)
Receive email from the division secretaries or CHC`s Admin. Assistant with the subject: XFEV Confirm.

3. Two e-mail files will be sent
a. P-Stamp Notification
b. Faculty Evaluation Print File
4. The Faculty Evaluation Print File has an attachment eval.out
5. Open attachment eval.out.
6. Click Save As
7. Save in: Comp_Center on `sbccddnas`(I):, Scannable Application
8. Go to Choose Programs and execute NCS Design Expert 3.1
9. Choose Open, Look in Scannable Application
10. Choose evaluate2
11. Choose Tools, Merge Wizard
12. Choose Comma Separated Text, Browse
13. Choose Files of Type, All File
14. Choose Eval (this is a Type: Out File)
15. Choose Open, Next
16. Choose Auto Match, Add Link, Next, Print
17. Make sure the printer is \\Galaga\b113-hp5si-ps
18. To place forms in the printer tray:
a. Make sure that the label "FRONT" and the scanning marks on the front of the forms
are facing you and on the left side.
b. Flip the forms over, like you are turning a page, so that the "FRONT" label and
the scanning marks on the front side of the forms are face-down and on the right
side.
c. Place the forms in tray #2.
19. Choose Print, you will see a progress bar and it will start printing.
20. An estimate of the number of forms to be printed:

Deliver to SBVC and/or CHC Instruction office: Printed evaluation forms, boxed by division with report "Faculty Evaluation "P Stamp" email, edited to include total forms on each email. This email includes Faculty Name, Division Code, Course-ID, section number, and number of forms printed for each section and a total form count.
